PRIOR TO YOUR APPOINTMENT

1. Fill out the  New Patient Forms. Download and fill out the form ahead of time helps speed up the checking process. If you prefer to fill the forms out at our office, please arrive 15 minutes before your appointment time.
2. Transfer your dental records. Contact your previous dental office and have your dental records sent to our office prior to your appointment.
3. Confirm the Appointment. Contact us and verify your appointment time 48 hours in advance.  We offer appointment reminder via text messaging two hours before your appointment time.

ON YOUR APPOINTMENT DAY

Bring the followings with you:

• Insurance card
• Photo ID
• List of currently taking medications
• New patient forms if they are pre-filled
